Assistance Programs
Hundreds of cases of blindness could be prevented each year if people were informed and followed proper prevention measures. Too often it is believed that visual problems will correct themselves. NCSS's Prevention of Blindness Program provides referral services to eye care professionals.
NCSS offers a Remedial Eye Care Program which assists low-income residents of Lycoming, Clinton, and Sullivan Counties with the cost of eye examinations and glasses. People in need of financial assistance for purchasing eye care services are encouraged to contact NCSS.
To submitt a referral, download the referral form. You will be contaced by a NCSS Associate upon return of the form.
Case Services
Once an individual is eligible for services, one of our qualified
caseworkers visits the "new customer" in their home to discuss agency services.
A case plan is developed with the NCSS Caseworker and the customer. A mutually
agreed upon plan is established and services begin. Each individual is assessed
and evaluated for specific services that are necessary to achieve that
individual's goals. |
